THE AWARD SEEKS TO HONOR AN INDIVIDUAL OF A HOME CARE COMPANY WHO HAS DEMONSTRATED EXTRAORDINARY SERVICE IN AN ADMINISTRATIVE POSITION 

This award is reserved for individuals currently working in home care in a support or administrative position and is open to all Provider TAHC Membership Types (Home Health, Hospice, Personal Support Service or Professional Service).

Examples of nominees: marketing staff, office manager, IT specialist, administrative staff

Consideration should be given to how the nominee has distinguished themselves in one or more of the following areas:

- Teamwork/staff relations
- Coordination of patient care
- Delivery of care
- Public relations
- Special event coordination/participation
- Agency management: financial, HR, etc.

Please answer the following (minimum of 150 words – maximum 500 words in total):
1. Give 1 to 3 examples of what specifically this employee did to go above and beyond for the customer (internal or external).
2. Was this action (or actions) by the direction of management or on the employee’s own initiative?
