This award is reserved for individuals currently working in a home care field leadership position and is open to all TAHC Membership Types (Home Health, Hospice, Personal Support Service or Professional Service).

Consideration should be given to how the nominee has distinguished themselves in the following areas:

• Teamwork/staff relations
• Coordination of patient care
• Delivery of care
• Public relations
• Special event coordination/participation
• Agency management: financial, HR, etc.
