THIS AWARD SEEKS TO HONOR AN INDIVIDUAL OF A HOME CARE COMPANY WHO HAS DEMONSTRATED EXCEPTIONAL SERVICE IN PROVIDING PATIENT/CLIENT CARE
This award is reserved for individuals currently working in the home care field in a clinical or paraprofessional position with direct patient or client care responsibilities. This award is open to all TAHC Membership Types (Home Health, Hospice, Personal Support Service or Professional Service).
Examples of nominees: Nurses, Therapists, Social Workers, Home Health Aides, Personal Care Workers, and Delivery Technicians
Consideration should be given to how the nominee has distinguished themselves in the following areas:
- Teamwork/staff relations
- Coordination of patient care
- Delivery of care
- Public relations
- Special event coordination/participation
- Agency management: financial, HR, etc.
